The Massachusetts Department of Mental Health, Metro Suburban Area
has established an innovative mental health-hospice and palliative care partnership to integrate end-of-life care planning into the mental health planning process.
On this Web site, this innovative program shares resources it has developed to:
- Elicit information about care preferences among patients with serious mental illness
- Assess capacity to select a health care proxy
- Assess educational needs of hospice and palliative care clinicians who care for persons with mental illness
- Teach mental health clinicians about hospice and palliative care
- Offer mental health administrators and clinicians policies and guidelines to support excellence in end-of-life care for their patients
